Create form.
Create what will link to the form.
Open form and go to the dropdown/checklist/radio buttons that need to be linked to.
Under Advanced, check Allow field to be populated dynamically and give it a unique ID (example: selectgame).
Change the following link to the site, page, unique ID, and item name as needed. If a single word, place single word. If there are space between words, put %20 between each word to function.
